Optimal Case Management LLC SMS Terms and Conditions

Optimal Case Management LLC operates SKEVAP, a patient-access and case-management application.

Program description

Users may voluntarily consent to receive one-time passcodes and verification codes from Optimal Case Management LLC for access to the SKEVAP application.

These messages are transactional authentication messages only. They are not marketing or promotional messages.

Optional participation

Providing a mobile phone number and consenting to SMS messages are optional. The applicable form can be submitted without providing a phone number or checking the SMS-consent checkbox.

SMS consent is collected separately from acceptance of these Terms and the Privacy Policy.

Message frequency and charges

Message frequency may vary based on the number of verification requests initiated by the user. Message and data rates may apply.

Opting out

Reply STOP to opt out. After opting out, the user will no longer receive messages through this SMS program unless the user later completes a new compliant opt-in.
