Optimal Case Management LLC SMS Privacy Policy

Optimal Case Management LLC operates SKEVAP, a patient-access and case-management application. This policy describes how information associated with the one-time passcode and verification-code SMS program is handled.

Information collected

A user may optionally provide a mobile phone number and separately consent to receive one-time passcodes and verification codes from Optimal Case Management LLC.

The mobile phone number is not required to submit the applicable form. SMS consent is collected through a separate, unchecked checkbox.

How SMS information is used

Optimal Case Management LLC uses the supplied mobile phone number only to provide transactional authentication messages, one-time passcodes, and verification codes for access to its SKEVAP application.

This SMS program does not send marketing or promotional messages.

Sharing and sale of SMS information

Mobile phone numbers, SMS opt-in information, and SMS consent are not sold, rented, or shared with third parties for their own marketing or promotional purposes.

Information may be provided to service providers that help Optimal Case Management LLC deliver, secure, and troubleshoot the requested authentication messages, or when disclosure is required by law.
